Ministry Interest Form

We are blessed to hear that you are interested in serving at CCCH! We want to invite you to grow in your walk with the Lord and become involved in the ministries and activities of your fellowship. We ask that all those interested in serving consider CCCH as their home church and have consistently attended in person for at least six months. 

Please note: this form is NOT an application for ministry but a form for reviewing the different ministries available to serve with. Once you submit this interest form, we will email you with follow-up information on applying for the ministry you have expressed an interest in.

Please check your email for follow up steps to complete the ministry application, background check and fingerprinting as part of the screening process to serve in the ministry(s) you have expressed an interest in. We do ask for a completed application for each of the ministries you are interested in serving in.
